One of the downfalls of the original instant replay was the length of the reviews. The first postseason controversy of IR occurred in the 1986 NFC Wild Card game between the Rams and Redskins, where a fumble by Rams TE David Hill took well over 4 minutes to review.
The official on the spot making the call was BJ Al Jury. The Referee was Gordon McCarter, Umpire in communication Bob Boylston.

Earlier in the game, Al Jury makes a DPI call the Jury also drops a USC. Pretty intense stuff.
