From the 1999-2000 playoffs. A scuffle ensues after an interception and McElwee and Leavy get roughed up. Washington's Tra' Johnson was given the boot for hitting Leavy in the head.

Shouldn't all penalties have offset?

Fouls committed against officials are the rare exception. They always get enforced, multiple times if necessary. The two unnecessary roughness fouls are dead-ball fouls, and offset. All fouls against an official are posted as between downs, regardless of when they happened.

Leavy really got dinged in the head. You can tell that really hurt.
