In hockey, there is what is called a gross-misconduct when there is head butting, or hair pulling during a confrontation and the offender is ejected.
Do you think if a player knees/kicks an opponent in the groin, gouges eyes, pulls hair, or basically tries to issue a cheap shot to hurt an opponet, should be ejected?

One of my mentors 22 years ago when I began officiating taught me four words... "obvious intent to injure." This was his guide as to when he ejected, and I've followed the same. All of what you listed, Fred constitute such an action as well. Disqualify for sure.
