Let's go back to 1995. Terry Bevington is the manager of the White Sox. Bevington turned out to be a joke, in way over his head.
The White Sox battery messes up on the very first pitch and hits Ken Kaiser on the head. Kaiser was mad about the situation and flipped a new ball back to the pitcher. On the second pitch of the game, you can hear Kaiser call "ball two" just as the pitcher releases the ball.
With that, Bevington starts getting on Kaiser for a lack of professionalism. Kaiser isn't in the mood for it, and tosses Bevington on the third pitch.
You can hear some NSFW hot mic comments from Bevington. Reading lips you can tell Bevington is telling Kaiser to do unnatural acts and accusing him of an Oedipus Complex. Kaiser also turns the air blue.
Perhaps one of the most astounding moments of this clip, is that Ken "Hawk" Harrelson, who hates umpires and blamed almost all White Sox woes on bad umpire calls, was laughing at this. The broadcast even said, with a chuckle, that the next pitches had to be "center cut" in order for Kaiser to call a strike. I don't know if Hawk didn't like Bevington and enjoyed Kaiser dressing him down, or Hawk liked Kaiser personally and gave him a pass.

Was there some sort of history between Kaiser and the White Sox and/or Karkovice? Kaiser clearly thought the head shot was intentional and responded in kind.

I don't think there was a history. Bevington always liked chirping at the umpires, so after getting hit in the head and hearing Bevington (who wasn't a respected manager) chirp, he decided to eject.
